When the doctors first told me that I had tested positive for a mutation on the BRCA1 gene, one year before diagnosis, my mind couldn’t take it in.

“We believe this gene is associated with the suppression of tumors, particularly in breast and ovarian tissue,” said the geneticist. “When cells replicate, some carry faulty DNA. If a woman has a mutation of this gene, the mechanism to clear up these faulty cells fails to work properly. This probably accounts for your mother’s death from ovarian cancer at 42.”

This was deeply confusing to me. My mother had had five children, very close together. Her life had not been easy. I think I had always felt that we were responsible for her death. Childcare and domestic drudgery had worn her out. But no, the doctors were telling me a different story.

“These cancers strike younger women. They run in families. It wasn’t until the 1990s that the BRCA genes and their role in the formation of cancer were discovered.”

I was 43 when I learned this, one year older than my mother when she died. It didn’t take a mathematical genius to work out that I was living on borrowed time. “What shall I do?” I said in stark terror. “My children are so little.”

The doctors explained that I had a 60% lifetime chance of developing ovarian cancer, which kills four in five women who get it. I also had an 85% lifetime risk of developing breast cancer.

They brought out a lot of graphs and showed me the pattern of how the cancers struck. A woman might go through her life to her late 30s or early 40s without knowing anything was amiss. And then all of a sudden, there it was, the black line setting off at a sharp sprint: lots of women developing breast and ovarian cancer.

Gravely, respectfully, the doctors explained that the only real way to protect against the ovarian cancer risk was oophorectomy, or removal of the ovaries, since there is still no reliable screening test for it, and it is often detected too late.

As far as I could see, it was a ‘no brainer’. And yet it was still one of the hardest decisions I have ever made. It felt literally ‘against my nature’. Even at the last minute, going down to the operating theater, I wanted to jump off the trolley. But I felt I had no choice. My children were so young, just two and four: I owed it to them to go through with it.

A kind nurse reassured me that she had seen others struggling in a similar situation. “You’re not alone,” she said. “I’ve seen other women going through the same thing. The only way they keep to their decision is by looking at photographs of their children.”

But when, alongside this, the doctors began telling me that my very high risk of breast cancer could also be managed by something called ‘bilateral mastectomy’, my mind went into freefall.

What? Were they seriously suggesting what I thought they were suggesting? What horrific idea was that? Was this some sort of sick joke?

I felt as if I had entered a horror film from which I would never emerge, like a nightmare you are conscious you’re having but can’t wake up from.

“Removal of both breasts reduces the risk of breast cancer by 90%,” they said. “A proportion of women with a BRCA mutation do opt for prophylactic surgery. And there are now a number of surgical ways of rebuilding a breast. Of course, it is a very personal decision.”

But I wasn’t having it. I was getting out of there. They could keep their statistics, their lifetime risks, their recently published papers on the increased survival of women who opted for surgery. They could keep their graphs about when the cancers struck in women with mutations of the BRCA gene and their terrifying euphemisms about ‘rebuilding a breast’.

How could you ‘rebuild’ a breast? It was a contradiction in terms, like taking a plaster cast of a humming bird, or writing a computer formula for a kiss.

No, no, no. I was off. I wanted no more of their world. I couldn’t even cope with what they had given me of it.

So I closed the case on mastectomy. I went about my business, which was hard enough. In summer 2005, there was oophorectomy, followed by crash menopause, with its welter of unwelcome symptoms. I stabilized on HRT, and got on with looking after my children. And then diagnosis forced the whole thing open again, in March 2006.

I couldn’t ignore the risk from breast cancer after all. It was real, not a fiction. The doctors were not sharpening their scalpels just to frighten me. They were sharpening their scalpels to try and save my life.

I was looking into getting myself a wheatgrass juicer and starting regular wheatgrass shots. But I’m wondering if I should bother. A juicer is a big investment and if spirulina does the same thing I’d rather save that money. Is wheatgrass better than spirulina?

An answer from Dr. Doug

In terms of nutrients, there’s not much comparison — spirulina definitely has more. For one, it has essential fats while wheatgrass doesn’t. Spirulina is also higher in calcium, iron, potassium, magnesium, phosphorous and zinc. It also has registered amounts of copper, manganese and selenium, while wheatgrass doesn’t have these in measurable amounts. It is slightly higher in vitamin C, E and choline, but spirulina beats it on the other B vitamins including folate.

But nutrients are only so important. There are other factors involved in our foods that can’t necessarily be quantified that need to be considered. So I did a little digging to see what I could find as far as studies on how these two green supplements actually affect us.

As I reported in my last post, the literature about spirulina shows that it boosts immunity, modulates the immune system, has anti-inflammatory properties, is an anti-viral, contributes to cardiovascular health by lowering blood triglyceride levels and is a potential anti-cancer nutrient, found to shrink tumors and prevent cancers from starting in rat studies. That’s a pretty impressive resume.

Wheatgrass, on the other hand, has antibiotic properties, is a blood builder and was actually found to be a viable alternative to blood transfusions in studies. It also has anti-cancer properties in the petri dish (not studied in live animals or humans that I could find) and reduces symptoms of digestive problems associated with colitis. It’s also an iron chelator, taking excess iron out of the blood in cases of iron-excess. Wheatgrass has also been found to have some alleviation of atherosclerosis, the hardening of the arteries associated with heart disease and was found to offset the unpleasant side-effects of chemotherapy in a study on breast cancer patients.

Given the varied yet impressive benefits between the two, the obvious answer would be to take both spirulina and wheatgrass! It doesn’t have to be at the same time — you could take one for a while and then switch to the other. Variety is the spice of health, after all. But if this doesn’t seem like a viable option, there is another factor to explore.

That factor would be the work involved in preparation. Wheatgrass requires that you either grow the wheatgrass or buy the cut grass regularly. Buying the cut grass isn’t very cost effective and, since growing it is pretty easy, this would be my recommendation. It grows fast and requires very little care. But this is still work (albeit easy work).

Then there’s also the work in actually doing the juicing and cleaning the juicer afterwords. Again, none of this is particularly difficult, but it is stuff that needs to be done and it takes time. You need to be honest with yourself about whether you’ll actually do all these steps on a regular basis or if this is something you’ll lose steam on, ending up with a juicer collecting dust in the corner of your kitchen. If you compare this to opening a jar of spirulina and dissolving it in a smoothie, juice or water, you see which comes out on top as far as ease of use.

And finally, you need to look at the cost. While wheatgrass is inexpensive on an ongoing basis (the hard wheat kernals needed to grow the wheatgrass are very cheap), it requires a substantial initial investment. Juicers that can handle wheatgrass (which use a masticating auger rather than centrifugal juicers) can be quite expensive. You can get ones that have hand-crank augers instead of motorized which are considerably cheaper, but then this adds to the work required. Spirulina, on the other hand, is not cheap on an ongoing basis, but requires no large investment to begin with. You’d have to do the math to figure out how long it would take doing regular wheatgrass juicing to pay for the cost of spirulina supplementing to see which one’s more worth your while. I figure it would take at least a year for the juicer to pay for itself; maybe more.

So either wheatgrass juicing or spirulina may be the best option for you. As with most decisions like this, it really comes down to preference. Do you like the idea of fresh wheatgrass juice, growing something yourself that you consume and that contributes to your health? Or do you prefer something quick and simple that you’ve bought from the store that fits into your schedule?

What to Eat After Wisdom Teeth Removal | Health Guides Daily

The wisdom teeth are the third set of molars in the very back of the mouth. A surgeon must often remove them because your mouth does not have enough space to accommodate them or because they come in at irregular angles that can cause your other teeth to shift. Sometimes wisdom teeth fail to erupt and stay trapped in the gums, causing pain or infection.

Having a wisdom tooth out can be a daunting and painful procedure. But happily, in most cases, the recovery period lasts only a few days. Your dentist or oral surgeon will prescribe appropriate painkillers and will give you advice on how to speed up the recovery process.

Although the pain may diminish after a few days, it may take a couple of weeks before your socket completely heals so it is necessary to stick to certain foods until it does. On returning home bite gently on the gauze pad every so often, and change the dressing if it becomes too blood soaked. The bleeding should stop after 24 hours if it hasn’t, telephone the surgery and ask them for advice.

What to Eat after your Wisdom Teeth are Extracted

  • You are going to be in a lot of pain for the first few hours so it may be advisable to stick to just clear, tepid fluids for the first eight hours. After 24 hours you may rinse your mouth carefully, using warm salt water to reduce the swelling and clean the wound. During the first few days after surgery you may not feel like eating anything much anyway so it may be advisable to stick to light food such as:
    • Ice cream
    • Frozen yogurt / non frozen yogurt
    • Cold applesauce
    • Cold pudding
    • Soft baby food
    • Scrambled eggs
    • Mashed potatoes
    • Gelatin
    • Thin soup (without the bits)

It is important that you do not disturb the clot that is forming in the empty socket so it is essential that you just stick to these soft foods initially. Eating soft foods such as this will stop the bigger pieces of food becoming stuck in the wound. Basically you need sustenance that can be merely swallowed with no need to chew. You can start adding solid food to your diet as healing progresses.

  • Stick to high energy, high protein drinks and NEVER drink anything through a straw. The sucking motion may disturb the clot. The same goes for people who smoke. You are going to have to refrain for at least 24 hours because that too can disturb the wound and lead to a condition known as “dry socket”. Believe me. The pain is unbelievable. So don’t invite it deliberately.
  • AVOID: Rice, nuts, grains, spicy foods and hard crusty foods that can scrape or damage the wound.
  • To ease the pain after surgery it may help to hold an ice pack on the outside cheek or personally I wrapped my hot water bottle in a towel and held this up against my face.
  • Physical activity may make the bleeding worse so take plenty of rest it’s important.
  • Sucking on ANY food can loosen the blood clot and delay healing, so make sure that if you eat a Popsicle to just let it melt slowly into your mouth.
  • Don’t keep feeling the area with your tongue, and touching it with your fingers is a bad idea, it puts the wound at risk from infection.
  • Keep your mouth clean by brushing your teeth and tongue carefully and after eating any kind of food rinse your mouth GENTLY with tepid salt water.
  • Having a wisdom tooth out can be quite a shock to the system. People cope with this operation in their own way but on the whole following these simple instructions will help you avoid problems in the future. If you are worried about the type of food that you are eating and whether it can damage your wound, why not do what I did?

Yes you’ve guessed it everything just went into the blender! After 10 days with a “dry socket”, I wasn’t taking any chances…

The best foods to eat are soft items that do not become trapped in the pockets created by the missing teeth. Your dentist will tell you how long you need to stick to a special diet.


Soft, stir-fried tofu flavored with garlic puree and soy sauce is easy to swallow and provides you with filling protein. Scrambled or soft-boiled eggs are another source of protein and nutrients. Refried beans and hummus are also soft and easy to swallow, and they offer both fiber and protein to keep you full. Cottage and ricotta cheese are a soft source of calcium for your post-wisdom teeth extraction diet.


Smooth, pureed soups such as tomato and butternut squash can be swallowed with no chewing. You may also be able to tolerate some Asian soups, such as egg drop or wonton. Avoid soups with large pieces of chewy chicken or meat and large chunks of vegetables.


Ice cream, gelatin, pudding and sorbet are soft foods that are easy to tolerate after having your wisdom teeth removed. You may also create smoothies using plain yogurt, frozen fruit and ice for a healthier sweet treat. You can also try custards and sweetened yogurt. Make sure all your choices are free of large pieces of nuts, fruit or candy bits.


Although chewy breads and crunchy cereals and crackers are poor choices after wisdom teeth extraction, you do have some starchy options. On a soft diet, you can include farina cooked with milk, noodles and pasta cooked until very soft, cereal—such as wheat or corn flakes—soaked in milk, soft flour tortillas, and the soft interior of bread.

Fruits and Vegetables

Bananas are naturally soft and provide nutrition. If chewing is painful, mash them or puree frozen bananas to make a mock ice cream. Many people enjoy eating mashed potatoes or whipped sweet potatoes after having their wisdom teeth removed. Applesauce and vegetable juice are additional fruit and vegetable options.

First off I have to compliment you on a great column. I read it religiously and, while I don’t always agree with your views, I am starting to see the light on better health.

I farm for a living and there are some deeply entrenched traditions in our diet that are not good and hard to give up. Making time to eat right can be a challenge, too. My problem and question is this: I am addicted to sugar. It is my drug of choice. Are natural sugars an alright alternative to sugar for me to get my fix? Honey and maple syrup (the real stuff from my uncle’s bush) are what I have been using in my one cup of coffee and tea each day. Just wondering if I am fooling myself with this logic.

Dr. Doug’s Answer:

I’m glad you’re enjoying the column! Eating right is a challenge for everyone, and being surrounded by family, deeply entrenched traditions and friends who don’t see things from the same perspective is probably one of the most difficult things to deal with when trying to make a change for the better. I feel the pressure myself by friends and family all the time.

Natural sugars are certainly a better choice than refined sugar. Maple syrup and honey (particularly raw honey) are sugars that still have mineral and nutrient components intact, and therefore the sugars enter the blood stream more slowly, avoiding spikes in blood sugar levels. Stevia is an even better choice, as it contains no sugar at all and only provides sweetness on the palate. This being said however, there is a difference between using a natural sugar as a treat or to add some sweetness to a dish when it’s needed, versus using it to feed an addiction.

From a holistic perspective, the fact that you require sugar on a daily basis is an indication that something is out of balance. The same can be said about people who require a cup of coffee to get them going in the morning, or an alcoholic beverage to “unwind” at the end of the day. Someone who is the picture of health doesn’t need any sweetener; in fact, they don’t need to feed any cravings at all. They may use a natural sugar occasionally to add sweetness, but it’s not a daily requirement, it’s a treat. I would say that you’re seeing the effects of a body that is suffering from some sort of imbalance and requiring sugar is a necessary means of coping with this imbalance. Correct the imbalance, kiss the sugar cravings goodbye.

The reasons for sugar cravings can be numerous, and I’m not going to try to cover all the possibilities here. My advice would be to see a holistic nutritionist or naturopath who would be able to get to the bottom of this health issue. They would be able to point out the choices in your diet and lifestyle that are contributing to your imbalance and be able to steer you in the right direction in overcoming it. They won’t solve the problem for you; the responsibility for making the changes is all still on you.

I know this probably isn’t the answer you wanted to hear, but I have seen over and over again the value of overcoming sugar cravings and coming out the other side the picture of health. As you return to a balanced state of health you’ll probably notice other nagging problems start to disappear as well

Burning, itching, sores, redness. If you have these symptoms in the genitals, you might have genital herpes. Most people with the disease do not know they have genital herpes, according to a recent study of 36 primary care physicians’ offices in six affluent areas. One-fourth of patients had the disease while only 4 percent reported they did. Join Douglas Fleming, M.D., a lead author of the study and assistant professor in the department of medicine at Robert Wood Johnson Medical School in New Brunswick, N.J., as he tells you what to look for and what you can do.

What is genital herpes?

Douglas Fleming: Herpes type 2 is the type that is caused by herpes simplex virus, commonly known as genital herpes, which often recurs throughout life. Almost everyone who has antibodies for type 2 has symptoms from time to time. It does not have a cure but does have effective treatments to manage and even prevent outbreaks. The great majority of people will “shed” the virus and may transmit it to other people.

The major symptoms of some herpes outbreaks are painful blisters or open sores in the genital area. This is the “classic” case of genital herpes. However, in reality, most outbreaks of genital herpes are not so obvious, and may manifest as itching or irritation in the genital area, discharge, pain with urination, or other symptoms that might be confused with other problems. For example, recurrent bouts of “yeast infections” or “urinary tract infections” may actually be genital herpes outbreaks.

The herpes sores usually disappear within two to three weeks, but the virus remains in the body for life and the lesions may recur from time to time.

Why look at affluent areas for this study?

Fleming: These results break the stereotype of who has a sexually transmitted disease. It tells us that genital herpes can affect anyone, even affluent populations. Most people assume STDs are contracted by low-income people or people engaging in activities that are high-risk. It’s not the case. Genital herpes, and some other STDs as well, are widely distributed in the population.

According to our study, 88 percent of people who are infected don’t know it. A study like ours certainly raises concern that we need to be more vigilant. Not knowing you have genital herpes can raise the risk of transmission.

What are the health risks of untreated genital herpes?

Fleming: Genital herpes can cause significant problems by causing devastating infections in newborns and in people who are immunosuppressed, such as AIDS patients or people with organ transplants. From my standpoint as an epidemiologist, it is very important to note that genital ulcers like those caused by genital herpes can it can make it easier to transmit or acquire HIV infection. So, herpes and HIV infection can work together.

Genital herpes that is active at the time of birth can be fatal to the newborn. If a woman’s physicians know about the infection, the woman can have a C-section to protect the child.

Why isn’t genital herpes identified better?

Fleming: Genital herpes can be mistaken for other problems. Genital herpes will have blisters that break down, and then there can be pain when urinating. For this reason, in women, genital herpes can be misdiagnosed as yeast infections or urinary tract infection. I always tell my medical residents to look for herpes if the patient keeps having recurrent infections, especially if the patient has unexplained symptoms.

The symptoms can be nonspecific. And people don’t think to look in their genital areas. So, when a doctor sees the scratched areas that are red and irritated, the doctor may not think of genital herpes as a possible cause. But doctors have to “think herpes” and do a culture or other test for herpes if genital herpes is to be diagnosed properly.

What are the symptoms of genital herpes?

Fleming: The classic symptom of genital herpes is a group of small, painful blisters in the genital area. However, only a few cases are this “classic.” Usually, a patient will just notice irritation or itchiness, or a little discharge. Men might think it’s a little knick in the skin, a zipper cut. Not everyone has a terrible outbreak. In fact, that’s a relatively rare way for genital herpes to present.

The sores usually disappear in within two or three weeks, but the virus stays in the body for life and lesions may recur from time to time.

How often does someone with the disease have outbreaks?

Fleming: It depends on the person. But usually, about 1 to 10 percent of their days include an outbreak, with viral shedding or symptoms or both. It’s not always possible to know when an outbreak will occur. It might be once a month or once every year.

How much of a problem is this?

Fleming: Our study suggests that genital herpes is a very widespread problem. It’s one of the most common STDs.

How does the virus act in the body?

Fleming: After it causes the classic blisters, the blisters clear up. The virus tracks up to the sensory nerve and lodges in cells near the spine. When the symptoms come back, it means the virus has gone back down the sensory nerve to the genital area from the spine.

Can it just clear up on its own?

Fleming: No. It usually lasts the person’s entire life, though recurrences are variable, as we discussed.

What treatments are available?

Fleming: First, I recommend getting to know what your outbreaks are like. Talk with your physician or health care provider about this. Recognizing an outbreak – and avoiding sex during the outbreak – is the best way to keep from infecting someone else.

There are antiviral medications that are very effective for genital herpes, and with relatively few side effects. They can be used in one of two ways. First, physicians can give a person having an active herpes outbreak an antiviral medication that will shorten the time to healing by a day or two. This can be very helpful, but generally not dramatic.

Antiviral medications can also be used in “suppressive” doses, which means that they are taken every day in order to suppress outbreaks from ever occurring. Used this way, these medications can reduce outbreaks by 80 to 90 percent. That’s a hugely dramatic effect. It cuts both symptomatic outbreaks and also cuts viral shedding that can lead to transmission of herpes to a sexual partner. Of course, the antiviral medications work only as long as the person continues to take them. They do not eradicate the virus from the body.

Question: Why do some foods cause bloating? If I eat fish and vegetables I’m not bloated. If I eat Indian food, I’m always bloated. Is it simply overeating? Is it food-combining? I would like to never be bloated again.

Feeling bloated means feeling gross. There are several things which could cause bloating and you guessed at two of them – overeating and food combining.

Basically, when you’re bloated it means that something has interfered with your digestion and isn’t allowing it to proceed as it normally would. Anything that interferes with the proficiency of digestive juices is going to make it seem like you’re too full. When food sits in the stomach for longer-than-normal, not digesting as it should, it can begin to ferment causing excessive gas which may increase the bloated feeling. Food that has passed from the abdomen to the intestine without being properly digested can lead to lower digestive bloating and gas.

Over-stuffing yourself is one example, where the acids in your stomach are overwhelmed by the shear amount of food that is there. Improper food combining is another possibility. Proteins, fats and carbohydrates all require different amounts of time to digest in your stomach before moving to the lower digestion. While carbs go through rather quickly, proteins undergo their main digestion in the stomach, taking longer. Fats take longest of all and can cause delays in stomach emptying which may increase the feeling of fullness.
